Pat Walker Health Center is committed to protecting the confidentiality of our patient’s medical and mental health information. We adhere to all federal and state laws governing the release of protected health information.

If you believe your privacy rights have been violated, or have additional questions, contact health center administration at 479-575-4077 or pwhc@uark.edu.

 

Parents/Guardians: Because federal law protects patient medical and mental health records, the health center does not disclose information to parents/guardians for students over the age of 18 without the student’s consent. Students can submit an Authorization for Release of Confidential Information form and list parent/guardian as recipient.

For students under the age of 18, a signed Authorization For Treatment of a Minor form must be signed by a parent/guardian for the health center provide medical care. 

 

Class Excuse Policy

The Pat Walker Health Center does not provide class excuses.

Because of federal health care privacy regulations, we cannot release protected health information (including dates and times services performed). 

If an illness prevents a student from attending classes for an extended period of time, the Dean of Students will be notified with the student’s permission.

Students and faculty may find it useful to use the Absence from Class Form as a means to have appropriate communication about class absences.
